Spring Valley Elementary School District is proud of our rich history. Many of our former students have accomplished great things after leaving our school district. If you or someone you know would like to share your success story and become part of our newly established Foundation Honors program, please complete the questions below and return this document. A list of honorees will be maintained on our District website. We will honor those successful former students with a link to their information. Those honored will also be posted in the school.

• Must have attended a Spring Valley Elementary school for at-least 4-years
• Must complete nomination form and picture
